WovenCare Advocacy Services supports individuals and families across Franklin County who are trying to make sense of complex healthcare and social service systems. We focus on practical guidance, compassionate communication, and trusted connections to the right resources.
Our advocacy approach is built for real-life situations: confusing discharge plans, difficulty accessing care, transportation barriers, housing instability after hospitalization, and family stress when decisions have to be made quickly.
This support is designed for Franklin County residents, caregivers, social workers, and community partners who need a trusted advocate to help coordinate care decisions and community-based support.
